Preoperative Glycemic Control

Background

Perioperative hyperglycemia is associated with increased postoperative morbidity that is reversible with improved glycemic control. Implementation barriers for perioperative hyperglycemia screening exist and constitute a know-do gap. Thus, the objective was to examine the effects of facilitated implementation of a screening protocol.

About the Project

ISQIC conducted a multi-institutional prospective cohort study using a quasi-experimental design. Through ISQIC a perioperative glycemic screening protocol was made available to member hospitals. A subset of rural hospitals received facilitation (support staff funding, peer-coaching, and benchmarked data feedback) for implementation of a of the protocol between October 2015 through April 2016. The project comprised of preoperative HbA1c patients with diabetes, preoperative glucose patients at high risk for hyperglycemia/diabetes (age >45 years or body mass index >30), and day-of-surgery glucose. The goal of the project was to examine the effects of facilitated implementation of a hyperglycemia screening protocol at such hospitals.
